Teenage female, anorexia, self-harming, cutting hospitalized, anxious, disrupted family, previous 4 years counselling ineffective

Introduction to Client

17 yo female with history of anorexia and self-harm (cutting) since 13. Parents extremely worried as situation is worsening and school work suffering. This being her final year at school with major exams approaching, mother concerned about daughter’s future. Client has been seeing counselors and psychologists for 4 years. Has been having weekly sessions with counselors for some time and has been highly anxious following these sessions and unable to cope with going to school the next day. Sleeps with parents as too frightened to be on her own at night. Family members all highly vigilant. Hospitalised recently following cutting episode and GP increased dosage of antidepressants. Parents at their wits’ end and have put off a planned overseas vacation for themselves.
Description of presenting problem

Client self-referred after speaking with mother. Client wanting to stop the cutting and anxiety/ panic attacks. Feels the only relief from the panic attacks is cutting. Nothing has helped in the past. Homework was presented to me at session one. History of bullying in primary school, repeating year 2 due to age (at the same school). Feeling guilty about her behavior and how it affects her parents and younger sibling.

The Presenting Problem

Client self referred after speaking with mother. Client wanting to stop the cutting and anxiety/ panic attacks. Feels the only relief from the panic attacks is cutting. Feeling guilty about her behavior and how it affects her parents and younger sibling.

Diagnosis

DAS session 1 extremely severe depression, severe anxiety and Stress. Homework, muscle testing and discussion following session 1 revealed secondary benefits for mental health issues.

Intervention

Classic TRTP (3 sessions, each a week apart) Session 2 pre session discussion using authoritarian approach and the secondary benefits that were evident. “Make a decision……”

Outcomes of Treatment

DAS normal prior to session 3

Client reports feeling happy, calm and feels in control of her life. Client reported this was the first experience of someone telling her to step up and take responsibility for her problem.

This client graduated from High School, and is nearing the end of her first year at Uni. Client lives away from home, has a part-time job and having so much fun. Client also reports that since our final session she has not “cut” and anorexia is history. All antidepressants ceased by client’s GP.
